Immunization Records

Ottawa Public Health is in the process of checking immunization records for 150,000 children in schools and licensed daycares in Ottawa. With the most up-to-date information about your child’s immunizations, Ottawa Public Health can ensure that all children in Ottawa are protected from disease outbreaks. If you are a parent or legal guardian, you may be receiving a phone call in the next weeks or months from Ottawa Public Health to make sure your child’s immunization record is complete.

Your yellow Immunization Record book has the dates of the vaccines your child has received. All vaccines need to be reported by the parent or legal guardian to Ottawa Public Health. If your child has received any new vaccines since they started kindergarten, please contact Ottawa Public Health with this information.

If you are not sure if your child’s vaccinations have been recorded in your yellow Immunization Record, please contact your health care provider to get that information and then share the information with Ottawa Public Health.
